Medicare Part D
Part D is private prescription drug coverage that pairs with Original Medicare. New for 2026: a $2,100 annual cap on out-of-pocket drug costs.
How Medicare Part D Works
Sold as private plans, with formularies that vary by carrier and pharmacy.
Sold Alongside Original Medicare
Stand-alone Part D plans pair with Parts A & B. With Medicare Advantage, drug coverage is usually built in.
Each Plan Has a Formulary
A formulary is the list of covered drugs. Your medications must be on the plan’s formulary to be covered.
Costs Vary by Plan and Pharmacy
Premiums, deductibles, and copays differ from plan to plan. The same drug can cost very differently at two pharmacies.
$2,100 Cap in 2026
Once your out-of-pocket drug spending hits $2,100, you pay $0 for covered medications for the rest of the year. This is brand-new.
Lifetime penalty for skipping Part D
Every month you go without Part D (or other creditable drug coverage) adds 1% to your future premium. Permanently. Even if you take zero medications today, signing up at 65 is almost always the right call.
